Bilingual Program Specialist, Case Administrator/Community Outreach

Child Development Institute (CDI) is a dynamic organization committed to the healthy emotional development for all children. We provide support, therapy and programs for children at risk of developmental delays and those of special needs and their families.
Responsibilities:
˙ Provide proactive primary case administration to clients/families including screenings, intake assessment, and support services
˙ Coordinate client care with medical, mental health and other providers
˙ Provide connection to CDI agency, community and government services and monitor referral completion including medical home, Regional Center and LAUSD
˙ Complete documentation and maintain database system
˙ Organize, create, routinely audit and maintain all department client files
˙ Oversee scheduling and following up on client appointments and referrals for medical and social services
˙ Develop relationships and maintain ongoing connection with clients and families during times they are not accessing services
˙ Develop and conduct surveys, compile data to assess the health needs of CDI Early Learning Center families and individuals
˙ Compile and maintain a comprehensive resource guide, for both print and online distribution, of service providers, community agencies, and partners
˙ Supervise and train volunteers, engage in interagency and parent-professional collaboration
˙ Promote effective and efficient utilization of clinical resources
˙ Mobilize resources and interviews, as needed, to achieve expected goal to assist in achieving desired clinical outcomes
˙ Communicate with clinical staff and volunteers on a regular basis and develop an effective working relationship. Assist physicians to maintain appropriate desired patient outcomes
˙ Serve as a client advocate. Enhance a collaborative relationship to maximize the client’s and family’s ability to make informed decisions
˙ Direct and participate in the development and implementation of client care policies and protocols in order to provide advice and guidance in handling special cases or client needs including IFSP’s (RC under 3), IPP’s (RC over 3) and IEP’s (schools age)
Knowledge, Skills and Abilities:
˙ Proficient bilingual Spanish/English oral and written language skills required
˙ Self motivated, detail oriented, flexible and highly organized
˙ Ability to produce well written reports and documents
˙ Ability to work well with others as part of a team and independently
˙ Excellent time management skills
˙ Professional experience in gathering and interpreting data
˙ Experience with excel data management
˙ Positively represents agency in community meetings
˙ Demonstrates initiative and shows high level of leadership
˙ Demonstrates professional level advocacy skills
˙ Demonstrate respect and sensitivity for cultural differences
˙ Demonstrates ability to prioritize competing tasks on a day-to-day basis
Qualifications:
˙ BA/BS or MA/MS in early child education, social work or related health or human service field
˙ At least three years of related experience
˙ Demonstrated ability to work in a family centered, collaborative way with parents, professionals and community members
˙ Proficient bilingual Spanish/English oral and written language skills required
˙ Strong oral and written communication skills and computer proficiency required
˙ Ability to work a flexible schedule, including some evenings and weekends

˙ Ability to sit for extended periods of time
˙ Ability to lift at least 50 pounds.
˙ Effectively use a computer and telephone to conduct business.
˙ Travel occasionally for meetings, community outreach events and seminars during the morning, day, and/or weekend hours.
